AUTHORIZATION AND RELEASE OF CLAIMS

 

I hereby authorize representatives of Atlas Public Schools to contact all persons and entities listed on this application and to make all other contacts, inquiries, and investigations that they deem necessary to verify my education, employment, criminal and child abuse/neglect history, and other qualifications for employment, including but not limited to contacting current and/or past employers, educational institutions, law enforcement, child abuse/neglect agencies, and other agencies or persons who knowledge about my fitness and ability to serve in the position(s) for which I am applying.  I hereby consent to the release of any such information by third persons and I understand that Atlas Public Schools will keep such information in a confidential file, available only to appropriate Atlas officials, and such other persons as may be designated by Atlas or who have access under the law.  

 

I hereby release and hold harmless the directors, officers, administrators, employees, and agents, including their attorneys and insurers of (1) Atlas Public Schools; (2) each of my past employers; and (3) any other person or entity providing information to Atlas Public Schools in connection with my application for employment, from any and all liability arising from disclosure of personnel records and/or from oral appraisals of my past performance or fitness for employment made to Atlas Public Schools.  

 

I understand that employment with Atlas Public Schools is contingent upon the satisfactory completion of an appropriate background check.  An unsatisfactory report shall constitute cause for rejection of an application or termination, if the applicant has been hired.  Although the existence of a criminal plea, conviction, and/or sentence alone may not constitute an unsatisfactory report, Atlas Public Schools has a compelling interest in ensuring the safety and welfare of its students, staff, and others present on its premises, and will make decisions that serve the bests interests of such persons and Atlas Public Schools.
